LCPlans DigitalVault Plan is a consumer-focused storage, management, and retrieval service for medical, legal, financial, and other personal confidential records and information. By becoming an account holder, you agree that:
The minimal personal information collected by LCPlans upon registration, including name, address, phone number, e-mail address, and credit or debit card, is collected for identification and billing purposes. Information beyond your name, address and phone number is optional.
As an LCPlans account holder, you are required to sign our HIPAA Statement. This authorizes us to release all medical and other confidential information you may provide us about you, in emergencies to appropriate emergency responders and to other people you authorize to view your documents and information during non-emergency situations.
LCPlans uses an online order form for account holders to purchase services. Information from the order form is used to ship orders, send information about LCPlans services and update an account holder's file. All account holders can review, update and modify their personal confidential information immediately through the secure website. Any changes entered by the account holder will be reflected in the database upon entry. You may also utilize our LifeMinder System to update your information on a periodic basis.
On our website, your account information, or the information for your dependent, can be viewed and/or changed only by an individual who is authorized by you. Account holders understand that LCPlans is not responsible for the validity or accuracy of any information stored by LCPlans. You may view and print your medical and legal documents and information from the LCPlans website; however, some of these documents cannot be edited or changed online. If you wish to add or make changes to your medical or legal documents stored in your LCPlans DigitalVault plan, you must create a new version of the document(s), and upload these new documents to your DigitalVault.
As a DigitalVault plan holder, you, your spouse/partner (when applicable) and dependents will have an identification card created that contains your name, identification number, medications, allergies, conditions and emergency contacts. By inputting the information into your DigitalVault account, the account holder has verified and confirmed the accuracy of all information on the card before printing and carrying it.
LCPlans understands that medical information can change anytime and it is the responsibility of the account holder to update their DigitalVault emergency information and print out a new card when there is a material change to their health information.
If your LCPlans Emergency Card is lost or stolen, we will change your specific identification number. Contact us immediately at firstname.lastname@example.org so that we can suspend access to your account until a new identification number is created for your account. You will be notified once your account is again active. You may then print out your new card with the new identification number.
If we are going to use your personally identifiable information, or that of a child's, in a manner different from that stated at the time of collection we will notify you, or the parent of the child, via e-mail. You will have a choice as to whether or not we use your information in this manner, or the information of a child. However, if you have opted out of all communication with the site, or deleted/deactivated your account, then you will not be contacted. Nor will your personal information, or that of a child's, be used in this new manner. In addition, if we make any material changes in our privacy practices that do not affect user information already stored in our database, we will post a prominent notice on our web site notifying users of the change. In some cases where we post the notice we will also e-mail users, who have opted to receive communications from us, notifying them of the changes in our privacy practices.
If an account holder, or a parent(s) or legal guardian(s) of an account holder who is a child wishes to terminate their account, or their child's account, they must do so by putting this request in writing and e-mailing it to email@example.com.
Upon receipt of the account holder's, or the account holder parent's or legal guardian(s), written request, the account holder will be forwarded a Termination form within 48-hours to complete, sign and return to LCPlans. Signing and returning the Termination form releases LCPlans from all future obligations and responsibilities related to the provision of that account holder's medical information for any reason, including emergencies.
Our policies provide for the safeguarding of account holders' information and prohibit its distribution (selling, loaning or renting) to any party except for the purposes for which an account holder has subscribed.
Your medical and legal documents and information, and any other account holder-provided emergency information, are available for retrieval via the LCPlans website. To retrieve these documents and information, the requestor must be authorized by you. Upon enrollment, an account holder authorizes LCPlans to release their medical and legal documents and information to any person or entity in possession of both the Account holder name and ID number (or access code).
LCPlans will release account holders' personal, identifiable information only when we have permission to share the information for the use it is intended which might include relaying information to emergency personnel in medical emergencies to assist with the speed of treatment of Account holders. Please refer to your HIPAA Statement cover sheet for additional details.
Although we make every effort to preserve your privacy we may need to disclose personal information when required by law where we have a good-faith belief that such action is necessary to comply with a current judicial proceeding, a court order, or other valid legal process.
In addition, if website visitors and account holders provide LCPlans an e-mail address, we will use it to send electronic announcements of special promotions, news and educational related items or general communications. This feature is only available to visitors and account holders who opt-in to subscribe to receive such information. Visitors or account holders who wish to opt-out can do so by un-selecting the check box. For a child account holder e-mails are directed to the parent(s) or legal guardian(s) of the Account holder.
LCPlans strictly protects the security of the information you provide. Personal information that we collect is stored in a password-protected computer database on servers with limited access. We carefully protect this information from loss, misuse, and other unauthorized disclosure, alteration or destruction. For more information, please refer to our Data Security Fact Sheet.
The LCPlans website does not store any personally identifiable information about visitors (non-Account holders) to our site. LCPlans website servers use log files. This information is not personally identifiable and may include number of visitors, user IP addresses, browser types, referring/exit pages, platform type, date/time stamp, and number of clicks. This allows LCPlans to analyze trends and gather data to help us to improve the web site. Log files are not tied to user's personal information.
When LCPlans Account holders visit the account services portion of our website to update account information, the website uses a technology called "non-persistent cookies". A non-persistent cookie is a token that a server gives to your browser when you access a website or a section of a website. Our server uses a non-persistent cookie when you are updating or renewing your account information to keep you from having to enter your password more than once during your visit to our site. This cookie is deleted once you leave our website.
This website contains links to other sites. Please be aware that LCPlans is not responsible for the privacy practices of other sites. We encourage our users to be aware when they leave our site and to read the privacy statements of each and every web site that collects personally identifiable information. This privacy statement applies solely to information collected by this web site.
When a parent attempts to sign up their child, the parent will be asked to provide his or her name, date of birth, e-mail address, mailing address, day phone number and credit card information, personal healthcare information, as well as to create a password.
We will not collect or retain any personal information from a child under the age of 13 unless you agree that you are the legal parent/guardian of the child you wish to enroll. In addition we will not collect any additional information other than that which will facilitate medical care in an emergency.
Information about children under the age of 13 whose parents have enrolled online will be used by Emergency Personnel to notify doctors, hospitals, emergency rescue teams, and family account holders that the child was found carrying an LCPlans Card. Their condition, medications, and dosages, and any other health issues will be conveyed to the above parties. Personal and health information will also be conveyed to the responder for the purposes of treating the child in emergencies situations.
We are required by law to take reasonable security measures to protect personal information about children. Registration information submitted by a child's parent cannot be modified except by the parent/guardian themselves that enrolled the child. All such information is contained behind secured networks and is only accessible by a limited number of employees and contractors who have special access rights to such systems. All contractors have signed confidentiality agreements with us. All credit card information supplied is transmitted via Secure Socket Layer (SSL) technology and then encrypted into our databases to be used and accessed only as described in this policy.
The legal Parent/guardian who enrolled their child with LCPlans online may also review or modify the information about their children within their electronic medical health record. Before disclosing information about a child, we may request that the parent provide reasonable information about the parent for identification purposes, such as the parent's name, address, phone number, credit card number, or other information.
Children under the age of 13 will never be asked to consent to disclosure to third parties, and their personal information will not be disclosed to any third parties other than the agencies listed above. A parent's consent to our collection and use of personal information about a child will never be treated as a consent to disclosure of that information to third parties.
If a parent wishes to terminate the child's account at any time this can be done online by completing the appropriate form for termination.
LCPlans may disclose personal information if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to the law or comply with legal process served on LCPlans; (b) protect and defend the rights or property of LCPlans; or (c) act in urgent circumstances to protect the personal safety of users of the LCPlans service, or the public.
